If you are purchasing more than one tribute ad, include them all on this form with one payment, but upload separately to the google form for each message.
Text and images must be uploaded to the Google form and payment received by Friday, November 3, 2023. Turn in the completed form, and cash/check for your Ad to your coach and/or leave at the UHS front office in an envelope, attn: UHS Boys Basketball Booster Club.
